BY BOOKING AN APPOINTMENT AT LOTTI LASH I AGREE TO THE FOLLOWING:

ALL DEPOSITS ARE STRICTLY NON-REFUNDABLE

All services offered at Lotti Lash are non-refundable. Although for any reason you are unhappy with your Lashes a free removal is offered within 24 hours of your service.

​

All transactions on any products purchased from Lotti Lash are final & non-refundable. 

​

Lash Extension Patch Test - $30.00.

A patch test is offered for those who have never had eyelash extensions before. It is a test to ensure you are not allergic to the adhesive that is used to apply lash extensions.

This service is not required to book in but a service offered if you are nervous of an allergic reaction occurring

 

Free removal is offered is reaction occurs after application of eyelash extensions.

Please do not rush your appointment. Full sets currently take me approximately 2.5-3 hours. This is because I take my time to ensure clean sets & the safety of your lashes. Please make sure you have enough time for your appointment as if I feel rushed I will stop the service immediately.

​

Please come to your appointment alone as I do not have a waiting area.

​

If you wish to reschedule or cancel your appointment please contact me at least 48 hours prior so I can fill your spot.

If your appointment is cancelled or rescheduled within 48 hours or you fail to attend your appointment your deposit will be forfeited as a cancellation fee.

 

In the case that your appointment was booked without paying a deposit, you must pay a 50% cancellation fee before you can rebook.

​

Not showing up to your appointment without any prior notice will result in a 100% fee of the service you booked.

You will not be able to book in again with Lotti Lash until this fee is paid.  

​

Please keep in mind our natural lashes shed approximately 2-5 lashes everyday (more in shedding seasons) although if significant loss of extensions has occurred within the first two days after your appointment , you may request a complimentary lash touch up. If a lash touch up is required it MUST be requested within 48 hours after completion of your service.

Anything after this refill prices will apply.

Eyelashes are constantly growing, shedding and replacing themselves with new lashes. To maintain the full effect, you will require regular refills every 1-3 weeks depending on your own lash growth cycle.  Anything over 3 weeks is considered a full set. 

​

 I understand as a part of the procedure eye irritation, eye pain, eye itching, discomfort and in rare cases infection may occur. I understand and agree that if I experience any of these issues with my lashes that I will contact my technician and have the eyelashes removed immediately and consult and physician at my own expense. 

​

I understand that even though the technician may apply and remove the eyelashes properly, that adhesive materials may become dislodged during or after the procedure, which may irritate my eyes or require further follow up care.

 

I understand there are risks associated with having artificial eyelashes applied to and/or removed from my natural lashes.
 

I understand and agree to follow the aftercare instructions provided by my technician. Failure to follow the aftercare instructions can cause the eyelash extensions to fall out prematurely. 

 

This agreement will remain in effect for the procedure and all future procedures conducted by Lotti Lash.

I understand that this agreement is binding and that I have read and fully understand all information listed above.

 

I represent that I am over the age of 18 years.

 

If below 18 years of age a parent or guardian must provide consent.

By proceeding with my lash appointment, I acknowledge eyelash extensions carry certain inherent risks including but not limited to redness, irritation, itching, swelling and / or other symptoms of ocular distress that are beyond Lotti Lash’s control and responsibility. 

​

I understand that in order to have the eyelash extensions applied to my eyelashes I will need to keep my eyes closed for the duration of 2.5-3 hours during the procedure. I also understand I will need to be lying in a reclined position. Any medical conditions that might aggravated by lying still for a prolonged period of time may mean I may not be able to have the procedure performed on my eyes.

This includes trimester 3 in pregnancy.
